    Program Coordinator (QDDP)

    Position Summary:
    Program Coordinator/QDDP-supervises waiver services to ensure health and safety. We are responsible for the development, implementation, and facilitation of waiver services for assigned individuals, ensuring provision of services as outlined in the service plan, that are responsive to consumer needs in regards to individual or group sessions. The Program Coordinator safeguards individuals by overseeing staffing as well as ensuring documents and services are in accordance with Health Standards and DHH regulations.

    Position Duties:
    • Read and review progress notes to ensure goals are being met and to ensure all documentation is current and in compliance.
    • Communicates with Support Coordinators regarding Individuals.
    • Supervises staffing and scheduling and ensures services are being delivered as efficiently as possible.
    • Attends in house meeting as well as community meetings and conducts monthly visits to individuals.
    • Assist in ensuring that DSWs are trained adequately to work in the homes.
    • Provide services to an individual when no back up DSW is available.
    • Provide and gather new information concerning the individual’s health, safety, future goals and achievements.
    • Observes individuals, reviews data and progress, and revises programs based on individual need and performance.
    • Ensure consistency among external and internal programs and disciplines.
    • Write Provider Plan of Care.
    • Performs other duties as deemed appropriate and as assigned by Executive Director.

    Candidates should possess the following:
    • Knowledge of Medicaid Home and Community Based Waiver
    • BS/BA Degree from 4-year college
    • Valid LA driver's license with Reliable Transportation
    • Auto insurance and current brake tag
    • Mature and dependable with good human relations skills
    • Team player that will adhere to mission of agency and advocacy
    • Willing to work flexible hours-some evening and weekend
    • Successfully pass Criminal Background and Drug Screens

    Lawn Care Worker

    Job Summary:
    Some Saturdays 9am to 1pm Responsible for the cultivation and care of the landscaping and grounds surrounding a business or building. Plants flowers, mows, pulls weeds, and maintains outside of building. You must have or be able to possess a Chauffeur’s license or obtain (within two weeks of hire) and pass a physical.

    Duties and Responsibilities:
    Perform groundskeeping and building maintenance duties.
    Mow lawn either by hand or using a riding lawnmower.
    Cut lawn using hand, power or riding mower and trim and edge around walks, flower beds, and walls.
    Landscape by planting flowers, grass, shrubs, and bushes.
    Apply pesticide to rid grounds of pests such as mosquitos, wasps, ticks, and others.
    Apply fertilizer to ground to enhance growth. Sweep walkway of debris.
    Cut down tree limbs that are posing a danger.
    Trim shrubs and pull weeds.
    Perform minor repairs and maintenance procedures on equipment utilized in groundskeeping.
    Rake, mulch, and prune the grounds as needed.
    Water plants and grass as needed and apply fertilizer.

    Job Coach/Driver

    Job Summary:
    Some Saturdays 9am to 1pm This job is with the vocational services department. You will be responsible for providing supports while providing habilitative and vocational services in the community. You must have or be able to possess a Chauffeur’s license or obtain (within two weeks of hire) and pass a physical.

    Duties and Responsibilities:
    Must have a clean driving record.
    Must have working cell phone
    Must be willing to work after 3pm and on Saturday
    Physically able to stand and walk for long periods of time.
    Be able to climb up and down stair, withstand hot temperatures, work outside in heat
    Must be pleasant in personality
    Able to be patient with people, not rushing to complete tasks
    Ability to repeat directives constantly
    Must be able to multi task
    Must be very adaptable
    Must be able to work with others in many different settings.

    Floor Maintenance Technician

    Job Summary:
    Maintains floors of facility to ensure a clean and well-maintained environment.

    Equipment used: Housekeeping cart, mop, bucket, various cleaning machines including but not limited to buffers, strippers, vacuum cleaners, and wet vac.

    Duties and Responsibilities:

    Vacuum rooms, common areas, halls and office with carpeting.
    Sweeps and mops tile and linoleum floors.
    Strips and waxes floors according to schedule.
    Buffs floors.

    Physical Activity Requirements:
    Employee is required to obtain the assistance of another employee when attempting to lift or carry objects in excess of 25 pounds. Employee must be aware that during the normal, routine performance of the essential functions, some of the following body movements may occur naturally, although they may not be a requirement of the job.

    Direct Support Worker

    Position Summary:
    Direct Support Worker is a person who assists an individual with a disability to lead a self-directed life and contribute to the community, assists with activities of daily living if needed, and encourages attitudes and behaviors that enhance community inclusion. A Direct Support Worker may provide supports to a person with a disability at home, work, school, church, and other community places. A Direct Support Worker also acts as an advocate for an individual with a disability in communicating their needs, self-expression and goals.

    Job Duties:
    • Responsible for maintaining a safe work environment by ensuring all equipment is in safe and operable condition.
    • Participate in all training and orientation sessions as directed.
    • Ensure all necessary paperwork is completed and turned in timely, neatly, and accurately.
    • Report any problems arising between the provider and the consumer and/or family members.
    • Treat all information concerning the consumer and/or their family as confidential.
    • Responsible to provide care to the consumer as it pertains to the “Plan of Care”.
    • Assist in the formulating of Service Plans/Plans of Care, etc. for each consumer in his/her charge, documenting progress or significant problems.
    • Advocate for services to enhance consumer’s life.
    • Follow all policies and procedures of the Arc of St. Charles. Review and comply with Consumer Handbook procedures.
    • Perform other duties as deemed appropriate and as assigned by Supervisor or the Executive Director.

    Minimum Qualifications:
    • Valid Louisiana Driver’s License
    • Valid Auto Insurance in your name
    • High School Diploma/GED/Documented work experience
    • Ability to read and comprehend basic instructions and complete specialized forms
    • Pass Screenings: Drug, Background, and Driving Record
    • Social Security Card
    • First Aid Certification-Class available upon hire
